Incomplete rebate claims will be rejected.
For each original purchase only one rebate per eligible pair of products is allowed. The consumer is responsible for any applicable taxes. Allow up to eight weeks from the date you mailed your rebated submission to receive your rebate. Rebate will be mailed to the address provided on the Rebate Claim Form. Lost, late, misdirected, torn, mutilated, incomplete, invalid, damaged, postage due, or illegible claims are not the responsibility of Mishimoto and will not be eligible for the rebate.
Mishimoto reserves the right to change the terms of this offer or suspend this offer at any time. Offer not subject to terms or claims of individual retailers. Offer not valid with any other Mishimoto promotional offer. All obligations of Mishimoto Automotive will be discharged by issuing the rebate and mailing it to the address you provide on the Rebate Claim Form.

- A completed original Rebate Claim Form. The original Rebate Claim Form must be filled out completely.
- The original Proof of Purchase from both the Mishimoto Silicone Hose Kit and the Liquid Chill product, which should be submitted in the form of the UPC removed from both the Mishimoto Silicone Hose Kit and the Liquid Chill. Submission of a rebate claim that does not contain the original UPC from both the Mishimoto Silicone Hose Kit and the Liquid Chill will be rejected, and the rebate will not be paid.
- A copy of the original sales receipt from your purchase of both the Mishimoto Silicone Hose Kit and the Liquid Chill, dated between January 27, 2012, and February 25, 2012.
